二、以下為健康檢查醫院,請擇一前往檢查(請攜帶入學健康檢查表)。
- 1.臺大醫院健檢:臺大醫院即日起至7月31日受理新生健康檢查,採線上預約制(健檢完成後需10-12天始可領取報告),預約網址:臺大醫院健檢預約系統,享有優惠價900元,限臺北總院,請留意健檢日期及報告領取時間,以免影響註冊時程。請詳閱「臺大醫院健檢流程及注意事項」。
- 2.市立醫院或勞工體格及健康檢查認可醫療機構健檢:健檢完成後需10-12天始可領取報告,若該院未提供口腔檢查,請攜帶檢查表至附近牙科診所補檢。
- 3.外籍生可在自己的國家的醫院完成健檢(檢驗單位需一致)或抵達台灣後完成健檢。
- 一般檢查--身高、體重、腰圍、脈搏、血壓、視(聽)力、辨色力、口腔及理學檢查。
- 尿液檢查--尿蛋白、尿潛血。
- X光檢查--胸部X光大片。
- 血液檢查--空腹血糖、三酸甘油脂、總膽固醇、高密度脂蛋白膽固醇、肝功能、肌酸酐、尿酸、全血液常規。
- 檢查前需禁食 6-8 小時(可喝少量白開水),前三天請勿劇烈運動。
- 視力檢查:有配戴眼鏡者,請攜帶眼鏡以便測量矯正後視力。
- 尿液檢查:用塑膠杯截取中段尿,再倒入尿管,如遇生理期,檢查前請先告知。
- X光檢查:檢查當日請盡量穿著無鈕釦、無金屬、無亮片的上衣,並取下身上的金屬飾品(如項鍊)。
- 如有懷孕,不需要做X光檢查,請先告知工作人員

六、補充說明
- 有效健檢報告必須為註冊日前3個月內由勞工體格及健康檢查認可醫療機構所開立者;若檢查項目不足,仍須補齊缺漏項目。
- 請妥善保存健檢報告(有效期限為5年),畢業後求職時亦可使用。
- 「僑生、陸生」需辦理「居留證(ARC)」者,建議抵達臺灣後至臺北市立聯合醫院和平婦幼院區辦理健康檢查(鄰近內政部移民署)。請攜帶「居留或定居健康檢查項目表」與「國立臺灣大學碩博班新生健康檢查表」合併辦理。
- 欲辦理休學之新生,仍須完成健康檢查後,始得完成註冊程序,人在國外可填寫「碩博班新生休學等特殊原因申請延緩繳交健檢報告申請書」。(適用對象:擬辦休學新生已於國外,或因病等不可抗拒之因素,無法完成健檢者。)
- 若無法於註冊期限內完成健檢,請於開學後儘速完成健康檢查,並先將健檢收據上傳至註冊系統或myNTU,可先行領取學生證;待收到正式健檢報告後,請儘速補上傳。
- 註冊系統關閉後仍需上傳健檢報告,請至myNTU搜尋「碩博班健檢資訊」。

◎碩博新生健檢管理標準作業流程
8月若因颱風天臺北市停班,當日原已預約臺大醫院健檢者,日後可憑預約資訊,週一至週三掛號時間內至1樓大廳現場掛號

National Taiwan University (NTU) Graduate Student Health Examination Instructions
I. Online Registration and Submission
Starting from August 6 (Registration Day), please log in to the registration system to complete the following:
- Health Information Questionnaire
- Ministry of Education Lifestyle Questionnaire

In addition, please upload your Health Examination Report.
II. Health Examination Locations
Please choose one of the following medical institutions for your health examination and bring the NTU Student Health Examination Form with you.
1. NTU Hospital Health Examination
NTU Hospital accepts health examination appointments for new students from now until July 31. Appointments must be made online.
- Health examination reports are available 10–12 days after the examination.

- Special student rate: NT$900
- Available only at the NTU Hospital Main Campus (Taipei).

Please carefully schedule your examination date and report collection date to avoid affecting the registration process.

2. Taipei City Hospitals or MOHW-Approved Medical Institutions for Occupational Health Examinations
Health examination reports are generally available 10–12 days after the examination.
If the medical institution does not provide an oral examination, please visit a nearby dental clinic to complete the oral examination section on the health examination form.
3. International Students
International students may complete the health examination either:
- At a hospital in their home country (all examination items must be completed by the same medical institution), or
- After arriving in Taiwan.

IV. Important Notes
- Please fast for 6–8 hours before the examination (small amounts of plain water are allowed). Avoid strenuous exercise for three days prior to the examination.
- For vision testing, students who wear glasses should bring them for corrected vision measurement.
- For urine testing, collect a midstream urine sample in a plastic cup and transfer it into the urine tube. If you are menstruating, please inform the staff before the examination.
- For chest X-ray examinations, please wear clothing without buttons, metal accessories, or sequins. Remove all metal accessories (e.g., necklaces) before the examination.
- Pregnant students are exempt from the chest X-ray examination. Please inform the staff in advance.

V. Follow-up Procedures
Re-examination notices will be mailed to your registered household address. For international students, overseas Chinese students, and Mainland Chinese students, notices will be mailed to the correspondence address provided.
Students may log in to myNTU and search for “Graduate Student Health Examination Information” using their NTU Computer and Information Networking Center (C&INC) account to check any required follow-up examinations.
VI. Additional Information
- A valid health examination report must be issued by a Ministry of Health and Welfare-approved medical institution for occupational health examinations within three months prior to the registration date. If any required examination items are missing, students must complete the missing items separately.
- Please keep your health examination report in a safe place. The report remains valid for five years and may be useful for future employment applications.
- Overseas Chinese students and Mainland Chinese students who need to apply for an Alien Resident Certificate (ARC) are advised to complete their health examination after arriving in Taiwan at the Heping Fuyou Branch of Taipei City Hospital (located near the National Immigration Agency). Please bring both the Health Certificate Form for Residence Application and the NTU Graduate Student Health Examination Form for a combined examination.
- New students applying for a leave of absence must still complete the health examination before completing the registration process. Students who are overseas may submit the Application for Deferred Submission of Health Examination Report Due to Leave of Absence or Other Special Circumstances.
- If you are unable to complete the health examination before the registration deadline, please complete it as soon as possible after the semester begins. Upload the examination receipt to the registration system or myNTU first to obtain preliminary approval and receive your student ID card. Once you receive the official health examination report, please upload it promptly.
- After the registration system closes, health examination reports must still be uploaded through myNTU by searching for “Graduate Student Health Examination Information.”
