『臺灣大學交換生健康檢查』流程及注意事項
◎ 檢查當日請攜帶健檢相關文件如下：
1.國立臺灣大學交換暨訪問學生健康檢查表點選連結
2.短期研修學生入境台灣之健康檢查表(丙表)點選連結
3.攜帶護照及2吋照片2張
註:註冊日未繳交健檢表，無法領取學生證。
◎健檢方式：
★ 臺大醫院網路掛號：
1、預約網址https://reurl.cc/18vryW
2、報到時間：上午8:30~10:30、下午13:30~15:00
3、未曾至本院就診者，健檢當日『務必』先至1樓大廳櫃台完成初診手續！！
◎健檢地點： 臺大醫院總院舊址2樓家庭醫學部門診15診隔壁健檢辦公室報到
(臺北市常德街一號，近捷運臺大醫院站2號出口)
◎檢查費用：新臺幣1000元整，於當天檢查時自行繳納。
◎檢查項目：
1、身體檢查：由護理人員量身高、體重、視力及聽力等項目，再至診間由醫師負責檢查。
2、抽血檢查：包含血液常規、肝、腎功能、尿酸、血脂肪及飯前血糖等項目（請空腹8小時）。
3、尿液檢查：尿液檢體請留取中段尿，若女性恰逢生理期，檢查前請先告知，一週內補送。
4、胸部X光檢查：穿著寬鬆衣物，避免穿戴金屬製品，若有懷孕請事先告知。
◎健檢報告領取：
1、報告需12個工作天(不含例假日)，健檢當日會通知可領取日期。週一至週五：早上10:00~12:00 、下午2:30~4:00。
2、自領：請攜帶健檢當日檢驗單及身分證備查。
3、代領：請攜帶雙方證件(身分證、健保卡或駕照，其一)及健檢當日檢驗單。
4、郵寄：採掛號寄送，需自備36元郵票(健檢現場無販售郵票，請事前準備)，健檢日後約3週收到(不含六、日)。

Procedures and Important Notes
Documents to Bring on the Day of the Health Examination
Please bring the following documents:
1. National Taiwan University Exchange and Visiting Student Health Examination Form
2. Health Examination Form for Short-Term Study Students Entering Taiwan (Form C)
3. Your passport and two 2-inch passport-sized photos
Note: Students who fail to submit the health examination form on the registration day will be regarded as not having completed the registration process and will not be able to receive their student ID card.

3. First-Time Visitors to NTU Hospital
If you have never visited NTU Hospital before, you must complete the new patient registration at the Information Desk on the 1st floor before proceeding with your health examination.

Health Examination Location
Department of Family Medicine, 2nd Floor
Old Main Building, National Taiwan University Hospital
(No. 1, Changde Street, Zhongzheng District, Taipei City)
Near MRT NTU Hospital Station, Exit 2.

Examination Fee
NT$1,000, payable on the day of the health examination.
Examination Items
1. Physical Examination
A nurse will first measure your height, weight, vision, and hearing, after which a physician from the Department of Family Medicine will perform the physical examination.
2. Blood Tests
The examination includes a complete blood count (CBC), liver and kidney function tests, uric acid, blood lipid profile, and fasting blood glucose.
Please fast for at least 8 hours before the examination.
3. Urinalysis
Please provide a midstream urine sample. Female students who are menstruating on the examination day should inform the staff beforehand and submit a urine sample within one week after their menstrual period.
4. Chest X-ray
Please wear loose-fitting clothing and avoid wearing any metal accessories. If you are pregnant or suspect that you may be pregnant, please inform the staff before the examination.

Collecting Your Health Examination Report
1.The report will be available 12 working days (excluding weekends and public holidays) after the examination. The collection date will be provided on the day of your examination.
Collection Hours (Monday–Friday):
10:00 AM – 12:00 PM
2:30 PM – 4:00 PM
2.In-person Collection
Please bring your examination receipt/slip issued on the examination day and a valid photo ID for verification.
3. Collection by an Authorized Representative
The representative must bring:
One valid photo ID for both the student and the representative (e.g., National ID card, National Health Insurance card, or driver's license), and The examination receipt/slip issued on the examination day.
4. Mail Delivery
Reports can be sent by registered mail. Please provide a NT$36 postage stamp on the examination day. (Stamps are not sold at the examination site, so please prepare one in advance.) The report will be delivered approximately three weeks after the examination (excluding Saturdays and Sundays).
