Health Examination Instructions for Incoming Undergraduate and Transfer Students

Instructions for the New Student Registration Document Verification System |
1. Please log in to myNTU and complete the "Basic Health Information" form and upload the health examination form (for students examined off campus).
Important: If you have only completed the "Basic Health Information" form, it is normal for the system to continue displaying "No Data." The information can be reviewed only after you upload the health examination form (for students examined off campus) or submit an application through the registration system to defer submission of the health examination report due to a leave of absence or other special circumstances.
2. New students, including transfer students, who participate in the health examination arranged by NTU do not need to upload a health examination form. Verification in the registration system will be completed at the health examination check-in desk. Students examined off campus must upload the health examination form or receipt during the online registration period.
3. Students examined off campus must upload a clinic or hospital health examination report issued within the past three months and covering all items listed on the NTU health examination form. If any required items are incomplete, complete the missing examinations and upload the full report. Reports issued more than three months ago are not accepted; a new examination is required.

Health Examination Information |
Pursuant to Article 8 of the School Health Act, schools are required to establish a student health management system and conduct health examinations for students. Accordingly, the NTU Health Center has invited an outstanding medical team from National Taiwan University Hospital to provide professional health examination services at a reasonable cost. The purpose is to safeguard the health of incoming students through early detection, timely follow-up, and treatment.

Please refer to the Health Examination Schedule. Students are assigned by college/department. No advance registration is required; attend at the scheduled time.

3. Important Notes Before the Health Examination |
3-1 Complete the Basic Health Information Form
Before the health examination, complete the Basic Health Information Form online using Google Chrome only. Failure to complete the form as required will be regarded as an incomplete health examination procedure.
Access path: myNTU Portal (https://my.ntu.edu.tw/) → Select "Student Area" on the left → Under Personal Information, select "Undergraduate Health Examination Information" → Log in with your NTU CINC account → Complete the Basic Health Information Form.
3-2 Identification Required at Check-in
Please bring your National Identification Card for verification at check-in. For non-Taiwanese students, the student ID number will be verified.

4. Important Notes on the Day of the Health Examination |
1. Fast for 6-8 hours before the examination. A small amount of plain water is allowed. Avoid strenuous exercise for three days beforehand, as it may affect the test results.
2. Vision Test: If you wear glasses, bring them so that corrected visual acuity can be measured.
3. Urine Test: Collect a midstream urine sample in the plastic cup, then pour it into the urine specimen tube. If you are menstruating, you must still provide a urine sample. Inform the staff when submitting the specimen so that a note can be made.
4. X-ray Examination:
1. On the examination day, wear a top without buttons, metal parts, or sequins whenever possible. If wearing a shirt, empty the chest pocket and remove metal accessories such as necklaces.
2. Because the X-ray vehicle has limited space and cannot provide a changing area, female examinees do not need to remove their bras. Standard underwire bras are acceptable, but do not wear shapewear with metallic patterns or fasteners.
3. If your hair extends below your shoulders, tie it up above shoulder level before entering the X-ray vehicle to prevent hair from interfering with the image.
4. If you are pregnant, you do not need to undergo the X-ray examination. Please inform the staff on site.

5. Examination Eligibility and Procedures |
1. The procedures for "Incoming and Transfer Students" and "Students Applying for a Leave of Absence" are shown in Figure 1 below.

Before the Examination 1. Attend the NTU Health Center at the time assigned to your college/department. Bring your National Identification Card; non-Taiwanese students will have their student ID number verified. 2. Students who need to change the examination time or complete the examination at an outside medical institution must apply by the deadline. Make-up sessions are at 3:20 p.m. on September 2 or September 3. 3. Students applying for a leave of absence who participate in the group examination must first pay the NT$900 examination fee in cash at Room 204 on the examination day. Complete the group examination, submit the health examination form at Final Review, and return to Room 204 to have the leave-of-absence form stamped. |

After September 15, visit the NTU Health Center, Room 204, to apply for a refund of the NTU examination fee. If the original report is not available during online registration, upload the health examination payment receipt first and upload the health examination report within one month. |

3. Returning students must download the NTU Health Examination Form (Chinese Version), complete the examination at a clinic or hospital, and upload the report to the online registration system. Alternatively, go to https://my.ntu.edu.tw/ → "Student Area" → "Undergraduate Health Examination Information."

6.Health Examination Fee |
1. NT$900 per person, collected together with tuition and miscellaneous fees. Students applying for a leave of absence must pay the fee on site.
2. Students who take the NTU health examination form to a clinic or hospital and complete the examination off campus may, after uploading the report to the registration system, obtain a refund application form from the Health Center after September 15 and process the refund at the Cashier Division.

7.Health Examination Items |
1. General examinations: height, weight, pulse, blood pressure, vision, hearing, oral examination, and physical examination.
2. Blood tests: complete blood count (CBC); hepatitis B tests (HBsAg, Anti-HBs, Anti-HBc); liver function (AST, ALT); uric acid (UA); creatinine (CRE); fasting blood glucose (Sugar AC); triglycerides (TG); and cholesterol (T-CHOL, HDL-C, LDL-C).
3. Urinalysis: urine protein, urine glucose, occult blood, and pH.
4. X-ray examination: chest X-ray.
5. All College of Medicine freshmen must undergo an additional color vision test assessed by an ophthalmologist. Examination time: August 31, morning only. Location: NTU Health Center.
Note: International students in the College of Medicine who complete both the residence health examination and the freshman health examination at Taipei City Hospital, Heping Branch, but do not receive the ophthalmologist-administered color vision test may come to the NTU Health Center at the time stated above.

8. Application to Defer the Health Examination Due to a Leave of Absence or Other Circumstances |
Students taking a leave of absence in order to retake entrance examinations are still required to complete the health examination and may not use this application.
Eligible applicants: students on a leave of absence, students currently overseas, or students unable to complete the health examination because of illness or other force majeure circumstances.

9. Follow-up After the Health Examination |
Health examination results will be mailed by mid-October to the household registration address. Results for international students, overseas Chinese students, and students from Mainland China will be mailed to the correspondence address. Students who do not receive a results notice may contact the NTU Health Center or log in to the myNTU Portal → "Student Area" → "Undergraduate Health Examination Information" and sign in with the NTU CINC account to view the results.
