The Quitline services, entrusted by the National Health Administration to Changhua Christian Hospital, have developed a structured smoking cessation education service model for adolescents. If your school has the need to refer students under 20 years old who violate smoking regulations for smoking cessation education, please apply accordingly based on the attached document.
Additionally, for schools applying for the required "Consent Form for Referral to Smoking Cessation Helpline Services" and electronic files for referral information (Excel file), the National Health Administration has placed them on the "Health 99 Website" under the Tobacco Control Hall/Smoking Cessation Services/Free Quitline. Please download and use them directly at the following website: https://gov.tw/x3m.
