因為臺灣大學保健中心於112年4月10日啟用全新的醫療系統(包含網路掛號及病歷電子化作業)，故需要重新填寫「電子式的初診病歷」，不論舊病人或第一次病人都要重新填寫。只要填過一次線上初診病歷即可。 填寫網址:
